2027 Medicare Advantage costs and benefits: what changed
Of the 4,445 Medicare Advantage plans that continue into 2027, 2,605 raised at least one copay and 1,660 dropped at least one extra benefit. Telehealth, meals and fitness coverage all retreated, and a quarter of the plans that gave back part of the Part B premium stopped. These are the headline figures from the Livmor Costs and Benefits Snapshot 2027, built from CMS's Plan Benefit Package data for 2026 and 2027.

Copays: mostly up
Among renewed plans, copay increases outnumber decreases more than three to one: 2,605 plans raised a copay on at least one Medicare-covered service, 807 lowered one. The biggest single move is outpatient hospital care, where the median copay rises from $350 to $400. Urgent care, ambulance, skilled nursing, surgery centers, labs, radiology, therapy, mental health, medical equipment and Part B drugs all moved on a meaningful share of plans.
The share of plans that charge a Part C deductible nearly doubled, from 567 plans to 1,120.
Extras: fewer plans offer them
1,660 renewed plans dropped at least one extra benefit and 267 added one. The retreat is widest in telehealth, which 97% of plans covered in 2026 and 82% cover in 2027, and in meals. The share of plans offering each extra, across all plans in each year:

Givebacks and flex allowances pulled back
The Part B premium giveback, a selling point on 1,529 plans in 2026, remains on 1,141 for 2027. Combined flex allowances, which pool several extras into one card, fall from 4,441 plans to 3,000. Riders and reduced cost sharing for chronic conditions both contract.
